Third Annual
"Best Shot for Kids" Golf Benefit


Get a team together to support kids with diabetes!
Date: May 12, 2008
Location: Highland Meadows Golf Club
Cost: $150 per golfer includes lunch, 18 holes of golf, cart, driving range and more.

Volunteers Needed: Registration (8 people), 50/50 tickets (2 person), Mulligan's (1 person), Betting Hole (1-2 people), Car Hole (4 people). Please call the DYS office, 419-480-1796, for more information or to volunteer for any of these positions.
Sponsorship Opportunities
Presenting Sponsor - $5,000 - 8 golfers, hole sponsor for 18 holes, and recognition in event publicity, at the outing and on DYS website
Gold Sponsor - $2,500 - 4 golfers, hole sponsor for 9 holes and recognition at the outing and on DYS website
Silver Sponsor - $1,250 - 4 golfers, hole sponsor for 3 holes and recognition at the outing and on DYS websites
Triple Hole Sponsor - $750 - hole sponsor for 3 holes and recognition on DYS website
Double Hole Sponsor - $500 - hole sponsor for 2 holes and recognition on DYS website
Hole Sponsor - $250 - hole sponsor for 1 hole and recognition on DYS website
