What Information Do We Collect?
Information is collected on this site in various ways. On the email contact form for Children with Diabetes, personal information such as full name, date of birth, address, phone number, and email are collected. In the Notes and Photos from camp section, a full name is collected as a part of the submission process. In the case of the Notes and Photos from camp, a signed parental consent form is required before we will complete the submission process and post the photos and notes to the Web site.

How Do We Use Personal Information?
Any information collected no the DYS Web site is used only for the purposes of providing services to Children with Diabetes and their families. No information will be released to any third party without your written permission.
 
How We Handle Changes in Policy
If the DYS ever changes the way it handles personal information collected from children under the age of 13, we will obtain new, signed parental consent forms outlining the change in policy.
