DYS United Way Designation Form
Dear Supporters of DYS,
This is the time of year when companies ask their employees to take part in the annual United Way Campaign. We would like to ask our donors, friends, families and board members to turn in the attached pledge form with your pledge card to designate that the funds you contribute come directly to DYS.
We have included the pledge form with DYS’ information, and we encourage you to consider designating all or part of your gift to DYS.

If you prefer to give online, you can designate your pledge to DYS simply by selecting “DYS” from the options in the designation field on the online form.
With your support, DYS can continue to offer educational and recreational activities to help local children that are living with Type 1 Diabetes. All activities are designed to promote confidence and teach techniques for managing diabetes while having fun and receiving the needed support.
